Regional Template for Syringe Driver Prescription, Administration and Monitoring Record
A request was made to the DHSSPS to develop a template for syringe driver prescription and administration. A multidisciplinary sub group of the NICaN Pharmacy Group and the NICaN Supportive and Palliative Care Group was convened. The purpose of the template was to promote standardisation, improve consistency in practice across N.I. and to improve document support. A regional scoping exercise identified at least 16 different syringe driver charts in use and widespread support for the development of a consistent approach. An extensive programme of consultation, focus groups and piloting was conduced within primary, secondary and independent hospice
sectors. Three templates were produced namely:
o Prescription and administration of medicines via subcutaneous syringe driver
o Continuation record for prescription and administration of medicines via subcutaneous syringe driver in primary care.
o Prescription and subcutaneous administration of medicines for breakthrough symptoms
in primary care.
in primary care.These templates were endorsed by the NICaN board in February 2009. The DHSSPSS is to commend these templates to the HSC as best practice guidance. Support available for the implementation of these charts has also been produced. This includes explanatory notes and a training package with speaker’s notes and slides.
